Once you have joined the plan, simply follow these directions and start using your membership today!

Please review your membership kit for details, as some discounts will have specific directions on how to use them.

Step 1: Schedule Appointment

Call or visit any of our locations to schedule your appointment. You can search providers to find the Family Smiles location nearest to you.

Step 2: Show your membership card

Present your member ID card at the time of service.

Step 3: Save

Pay the discounted rate at the time your treatment is provided. That's it!
